Change Requests DB

Is it possible to get a template for the Ninox Beta Team Change Requests DB? I'd love to have a similar resource in place for my team to manage development of our database.

    Thanks for response, but I just ended up creating a single table in my existing database for users to log requests based off of the one from the Beta Team. It's capturing everything I need so far. I'll reach out if I find it's not living up to expectations.
