Can I with a field from one database open field in another database?

I have 2 databases as attached. One with appointment and the other with medical record. I need to say that when I click Jose Pedro on the schedule, he takes it to Jose Pedro on the medical record. Waiting for an answer.

    @Lucidio, 

    The following data model assumes that you will create an appointment record and link it to the person.   Then, from that appoinment .. click the Create Record button.. and create a NEW Medical Record associated with the person.

    I couldn't solve the problem. I need guidance on how to access a second database from a name scheduled in the database. I click on the patient's name and open the Prontuario database on the patient's record to make the description.

     

    It could be a database with the names of the patients
